Monthly Plans:

• $50/mo per person   
• $125/mo per family (max 3, additional immediate family +$25/mo each)
• $25/mo for mechanic/ rig-driver/trainer with paying rider 
• $100/mo for mechanic/ driver/ trainer family (max 3)  

*I ask for a 6-month commitment billed up-front or monthly with 30 days notice to cancel. No hassle, no questions asked.

 Camper Plans

 (Open to ALL facilities): 

• $30/wk paid at sign-up, non-refundable 
• + $20/mo mechanic
• $75/mo month-to-month (max 3 months)
• + $25/ mo mechanic
• Visits w/o signing up for a plan at the beginning of the "camp": $60 (does not include follow-up visit/texts/emails).  
• Family discounts available as well. 

• Price locked in for 2 years. 
• **Limited spots available as I want to ensure I am able to give every patient the attention and time they deserve** 

• At cost labs: can be paid for on-line and drawn a local lab such as Quest or Labcorp. Standard labs are very affordable. 
• Imaging such as Xrays, CT, MRI
• Blood pressure cuff, glucose meter (if indicated) 
• Cost of medications (GoodRx is helpful): I am aware of the very cheap to free medication options offered by pharmacies and will prescribe them when appropriate. 
• Any special test that is ordered such as a sleep study or cardiac stress test. Again, places offer these for a discounted cash price. 

Insurance Benefits

•  If you have insurance, many of these services (except my fee) could be covered depending on your coverage.
•  Occasionally a claim for a service will be denied until they verify that I am a DPC provider and then they will go through. I will answer these requests ASAP.  
• Does not apply to Medicaid or Medicare (see DPC section).

Discounted Services 

• Contracts with Quest and Labcorp provide you with very affordable general labs for ~$5.
• Local office provides use of their x-ray for a very reduced price.
• Most advanced imaging can be ordered for a significantly reduced price at local imaging centers.
